It contains important information regarding the upcoming flu vaccination catch-up session at your school and instructions for completing the online consent form.

Message for Parents/Guardians
Subject: Flu Vaccination Catch-Up Session – Important Information
Dear Parent/Guardian,
Our Immunisation Team will be returning to your child’s school on 14th November 2025 to offer another opportunity to receive the flu vaccination.
If you haven’t already, please complete the online form to record your wishes:
- To give consent for your child to receive the flu vaccine, please click here:

- To decline the vaccine, please still complete the online form. This helps us update our records and avoid sending further reminders.
You only need to submit one form. Your consent (or refusal) will stay in place for the whole flu season.
Important
If your child has already received the flu vaccine during our previous visit in September 2025, or at another vaccination clinic or school, please do not complete another consent form (either paper or electronic).
If your child was absent or refused to have the vaccination during the first visit, we will offer the vaccine again during the catch-up session.
If you no longer wish your child to receive the vaccine, or they have been vaccinated elsewhere, please contact the Immunisation Team on 0203 343 2400/havering@v-uk.co.uk
A gelatine-free injectable vaccine is also available upon request.
