Dear Parent/Guardian,
The flu vaccine is being offered at your child’s school again this autumn/winter.
We are at Brady on the Monday 23rd September 2024.
Please complete the online form so that we can record your wishes.

To decline the vaccine, please complete the online form so that we can record your wishes.
You only need to submit one form. Your consent will remain in place for the entire flu season. If your child misses a first visit, you do not need to complete a second form.
